We understand that your pet’s surgery day can be a stressful and busy morning. To help speed up and take some of the headache out of the process, you may fill out the Surgery Check-In form below and have it submitted electronically so everything will be ready to go when you drop your pet off for surgery!

In keeping with Midway’s tradition of providing the most up-to-date quality medicine and surgery available, many options are available for our surgery patients. After completing and submitting your form, a detailed procedure estimate can be emailed to you if you desire. Just click on the “Please send me an estimate” box at the end of the form.
